Non-Hodgkin lymphoma (NHL) is a heterogeneous group of lymph proliferative malignancies.

The development of NHL is an extremely complex process, and the etiologic heterogeneity among subtypes has been noted. Despite being generally chemo sensitive diseases, the last decade has focused on developing more targeted agents based on improved insights of underlying biology. There is need of more rational treatments to improve both the efficacy and toxicity profile of standard approaches and to improve clinical outcomes. (2)

In Ayurveda, rasayana therapy is one of the main branch of eight branches of Ayurveda which is multidimensional therapy. It’s possible mechanisms in cancer management  may be by various its properties like immune stimulation, antioxidant, quenching free radicals, increasing cellular detoxification, repair damaged non proliferating cells, inducing cell proliferation, and self renewal of damaged proliferating tissues, and replenishing them by eliminating   damaged or mutated cells with new cells. (2)

Many in vitro–vivo studies also reports anti tumour, pro-apoptotic, anti proliferative and anti migratory activities of many Ayurvedic medicines. These all drugs act as an important adjuvant to chemo-radio therapy and enhance quality of life of patient. (2,3)

Conventional modalities of cancer like chemotherapy and radiotherapy are highly toxic and both damage adjacent healthy tissues. Most of the patients suffered from adverse effects of these treatments. Some important side effects of chemotherapy are nausea, vomiting, alopecia, diarrhoea, constipation, mucositis etc. Radiation therapy can produce systemic side effects like anorexia, nausea, fatigue, alteration in the taste, anaemia, dry skin, constipation etc. Late complications of these therapies also include pharyngitis, esophagitis, laryngitis, persistent dysphagia, fatigue, hepatotoxicity, infertility and cognitive deficits. There is also a possibility of development of secondary cancer due to chemoradiotherapy. (2)

These all above known or unknown side effects have devastating effects on quality of life of cancer patients. Hence considering all these probable adverse effects of available conventional treatments, in presented case we had planned herbo-mineral treatment to maintain quality of life of patients and make her able to tolerate chemotherapy cycles well.

In our Rasayu Cancer clinic we successfully treated many cancer patients with Ayurvedic rasayana therapy on the basis of basic Ayurvedic fundamentals and Arbud (cancer) pathology given in Ayurvedic texts.

The current study reports the case of a patient with NHL treated with multiple lines of treatment. This patient was treated by only Ayurveda after completion of her chemotherapy cycles. After starting Ayurvedic treatment she had showed favourable response to Ayurvedic therapy.

Case Presentation

A 42 years old female patient was presented to a secondary care hospital with history of fever, cough.She felt lump in her axilla. On examination doctor felt enlarged axillary lymph nodes. He advised Histo-pathological examination which was performed on 12-11-11. Histo -pathological report revealed Non lymphocyte predominant Hodgkins Lymphoma. Second opinion biopsy report dated 01/12/2011 showed Lymphoid nodules rich in B lymphocytes as marked by CD20 .Interfollicular cells and cuff off lymphoid cells are CD3 positive. After that patient underwent for CT chest/Abdomen /pelvis on 21-11-11, which reveled multiple lymph nodes in bilateral axillary regions consistent with NHL. Air taping and Mosaic perfusion was also seen in both lungs. She was also suffering from anorexia, nausea, and general weakness.Oncologist had advised her chemotherapy. Hence, Patient came to our clinic for Ayurveda treatment on 28/11/2011.She decided to start our treatment with an aim to get relief from symptoms and to improve tolerability to chemotherapy. Immediately we started anti malignant rasayana therapy with supportive medicines as per given (Table:1)on same day for a month.Bone marrow study was performed on 16-12-11 which showed uninvolved Bone marrow.Patient was advised 6 cycles of Chemotherapy with combination of Adrimycin 40 mg, Vinblastin 9.6 mg and DTIC 600mg. After a month patient came for follow up with relief from symptoms. By seeing this response we continued treatment. The patient was absolutely stable for 30 months after starting rasayana therapy and was not having any physical complaints. Her tolerability to chemotherapy was increased. She had completed 6 cycles of chemotherapy in between 10/01/12 to 25-05-12.All the cycles of chemotherapy were well tolerated by the patient without any signs of toxicity of Myelosuppression. Post two cycles of chemotherapy along with rasayana therapy CT scan (09/02/2012) was done for evaluation of tumour size which revealed decrease in size of bilateral axillary lymph nodes. Left axillary collection was regressed.Further CT Scan dated 12-04-12showed further decrease in size of lymph nodes and no new lesions noted.PET scan dated 06-07-12 reveled evidence of residual active disease in multiple metabolically active bilateral axillary lymph nodes. No evidence of metabolically active disease seen elsewhere in the body.After this the patient started Radiotherapy along with Rasayana therapy.PET scan dated 19-12-12showed reduction in size and resolution in size of bilateral axillary lymph nodes, with no significant residual activity in residual nodes. No evidence of metabolically active disease seen elsewhere in the body. Patient was almost symptoms free and her quality of life was also excellent. During this 30 months treatment period she didn’t experience any adverse effects of this Rasayana therapy.

Outcomes

  • This case showed excellent tolerability to CT and RT in combination with Anti malignant Rasayana therapy No side effects other than mild decreased Hemoglobin seen.
  • Anti malignant Rasayana therapy in combination with CT and RT helped in tumor regression and improving QOL.

Discussion

With available conventional chemotherapy and radiotherapy leads to a satisfactory outcome in patients with this uncommon neoplasm, which tends to have a favourable prognosis but some NHL patients relapse or experience adverse events from treatment.

Quality of life of these cancer survivors is worst. Rasayana therapy can be helpful in the management of cancer many ways .It has been using since several years for Adjuvant or co-therapy along with chemotherapy or radio therapy and Post surgery care in cancer to minimize the side effects of these therapies and can be helpful in targeting the specific tissues as in some studies on nano-particles of gold. 4

Therefore it is needed to develop new anti malignant rasayana therapy approach to prevent adverse effects of chemo-radiotherapy and for such patients who would not be tolerating or having resistant to chemotherapy.

In our Rasayu Cancer clinic we successfully treated many cancer patients with Ayurvedic rasayana therapy. Considering limitations of available conventional therapies, there is need for incorporating Ayurvedic principles or medicines with mainstream cancer medicines.

The Ayurvedic compounds were selected so as to support and restore the normal functions of the Lymphatic system and to relieve patient symptoms and to regress patient’s axillary mass. It mainly includes Navjeevan Rasayana-a proprietary medicine (Swarna Bhasma – Bhasma (Calx) of Gold, Rajat Bhasma, Hirakbhasma-calcinoiddiamond, AbhrakBhasma, -calcined Mica, Triphala powder etc) with other symptomatic medications as mentioned in Table no-1.

In this case patient showed excellent tolerability to chemotherapy and radiotherapy in combination with Anti malignant Rasayana therapy. No side effects other than mild decreased Hemoglobin was seen. During her 30 months treatment period she didn’t experience any adverse effects of this Rasayana therapy. This showed safety and efficacy of rasayana therapy and other Ayurvedic drugs in cancer disease.

Conclusion

Ayurvedic anti-malignant rasayana therapy is safe and efficient treatment for NHL patient to prevent adverse effects of Chemotherapy and Radiotherapy. It can helpful in improving four domains of quality of life of cancer survivors. There is further need to develop new cancer treatment protocol with incorporating Ayurvedic treatment to improve overall success in cancer treatment.
